Memantine HCl
Indications/Uses
Moderate to severe Alzheimer's disease.
Dosage/Direction for Use
Adult Treatment initiation pack: Initially 5 mg/day, stepwise increased over the 1st 4 wk. Dose titration: Wk 1 (day 1-7): 5 mg/day for 7 days; wk 2 (day 8-14): 10 mg/day for 7 days; wk 3 (day 15-21): 15 mg/day for 7 days; wk 4 (day 22-28): 20 mg/day fro 7 days. Max: 20 mg/day. Maintenance: 20 mg/day. Elderly >65 yr 20 mg/day. Renal impairment, moderate (CrCl 30-49 mL/min) 10 mg/day, if tolerated after at least 7 days may be increased to 20 mg/day; severe (CrCl 5-29 mL/min) 10 mg/day.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to memantine HCl & other pyrrolidone derivatives.
Special Precautions
Epilepsy (history of convulsions or w/ predisposing factors). Avoid concomitant use of N-methyl-D-aspartate (NMDA)-antagonists eg, amantadine, ketamine or dextromethorphan. Monitor for some factors that may raise urine pH. Patients w/ recent MI, uncompensated CHF (NYHA III-IV) or uncontrolled HTN. Fructose intolerance (contains sorbitol). May impair ability to drive or operate machinery. Not recommended in severe hepatic impairment. Not to be used during pregnancy & lactation. Not recommended in childn <18 yr.
Adverse Reactions
Drug hypersensitivity; somnolence; dizziness, balance disorders; HTN; dyspnoea; constipation; elevated liver function test; headache. Depression, suicidal ideation, suicide.
Drug Interactions
May increase effect of levodopa, dopaminergic agonists & anticholinergics; reduce effect of barbiturates & neuroleptics. Concomitant use w/ antispasmodic agents, dantrolene or baclofen. Increased risk of pharmacotoxic psychosis w/ amantadine, ketamine, dextromethorphan & phenytoin. May increase plasma levels w/ cimetidine, ranitidine, procainamide, quinidine, quinine & nicotine. May reduce serum level of hydrochlorothiazide. May increase INR w/ oral anticoagulants eg, warfarin.
